1. load an image
2. add a Side Branch
3. add the Monochrome transformation to the image above the splitter, not to either of the branch boxes, leave the Monochrome dialog open.
4. click on the empty branch boxes (not the on the splitter) under the imput image -> the preview shows three distorted channels, or one channel three times, next to each other
5. click OK or Apply -> the branch boxes previews are OK.
Also closing and reopening the Monochrome after the step 4 will cause the splitter and the branch boxes to be processed and the problem will not reappear.
To replicate the problem, you need to close the Monochrome thumbnail together with the splitter and the branch boxes and repeat the step 2 and 3.
Another observation:
1. with the Monochrome dialog open, add the Color Balance to the empty branch boxes -> you get the message Internal error: zoom factor = 0
2. keep pressing Escape key until the Color balance dialog opens
3. close Color balance and now you can add it again without the error message

It looks like the problem is even more widespread than what you reported. It seems to be related to the bypass button being grayed (all side-branches have bypass grayed even though it is not displayed).
