At first I thought is was the particular sequence of transformations, but this happens even without performing any transformations:
Open image - one that was originally shot with a vertical camera orientation
"Save Back" (or perform a series of transformations, then Save Back...)
Image is saved in a rotated orientation, and when the embedded script is opened, an extra image has been added to a side branch:

I copied the code that handles this for TIFF files to JPEG and downloaded a suite of JPEG test files in every one of the 8 possible orientations which all seem to open OK, so the problem may be fixed in the next release.
If not, different cameras might set the orientation code differently - could you email me a sample rotated image. Also please try rotating the camera both ways (left and right) as this can generate different orientation codes as well.
