I'm not sure if that has been suggested before - it's about manually smoothing e.g. skin imperfections, but retaining realistic texture, akin to Magic Brush/Healing Tool, or whatever it is called elsewhere. I just discovered this being compelled to beautify a portrait...
- Duplicate the original image (Geometry/Copy)
- Composite, with the Original image as the Input Image and its copy as the Overlay; set the Preview to Auto and view it in 1:1; set Operation = Low Pass
- Use Speck Removal Tool on the copy of the original (i.e. the Overlay), observing changes in Preview; set the Speck Removal brush transparency and softness to some high values; adjust the Blur Radius in the Composite to control the coarseness of textures preserved
- Adjust Composite Amount (and Overlay Amount)
If it works for others and if someone could illustrate it (I can't at the moment), perhaps it's worth putting in the Tips section.

That sounds like an interesting technique.
Another technique to consider is noise reduction. There is an example in the PWP manual.
